98 pcm reporting "not ready" at emission testing.

Does anyone know what this might be. I have tried changing some of the conditions for dtc and it makes no difference. When I click on system readiness it gives me a not ready for catalyst, O2 sensor, evap, and air pump. The same as the emission station readout. Could I have a ground problem?

Stock except for long tubes and exhaust. Did not have this problem last year. There were some dtc set which I cleared and got it passed. Could running open loop cause this...like a general default? Thanks.
