I have a LQ4 was a DBC but I converted over to a DBW with a TAC controller part number 12588923 and its pedal assembly. It works till it hits 85% throttle and then it closes the throttle blade and will not open till you cycle power on the ECM. I did a complete reflash of the ecm with a 2003 LQ4 and DBW. Also it has an idle issue but the engine is all stock. I have added the tune file and a log file if anyone can look at them and show me what might be going on with it.

Check what DTC P1518 means (seen in your log).

It seems like the TPS and ETC sanity tables may be being exceeded (see tables in Engine Diagnostic segment).

For DBW, log the pid GM.ETCTP.

I see in your log where the throttle is shutdown at 85%... but the RPM has already dropped to zero before this... you have other additional problems.

I have figured out the problem with the DBW. I was using a Red/Blue ECM and a TAC and Throttle body from a Green/Blue ECM. Once I reflashed a Blue/Green ECM it started working correctly.
