help with p0101 code, tune file attached

The car has a 6.0 H/C/I with a GM 85mm MAF. I'd say about 75% of the time i am shifting through the gears under light throttle (10-15%) and most often in 3rd gear around 3k rpms the car will hesitate and then pick back up, then sets a P0101 code. It also sometimes does it when going up a slight incline with constant throttle.

I've had my tuner look at it and he can't seem to find the issue. It never does it under full throttle either.
