I am also curious how you would convert a DBW truck to cable? Not sure if that is what the OP is trying to do but I can think that it might make a few headaches but solve a few as well.
For an 04 DBW truck use the 04 COS3 and dont change anything. Use another 04 OS if you have to so you can run a COS, but there are 4-5 COSs available for 04 PCMs.
03+ trucks are all DBW so if you want to go to a DBC setup then you'll need a throttle body, PCM, TAC, and maybe a pedal assembly. Honestly not worth it, the changeover from the 99-02 512kb PCM to the 03-08 1MB PCM includes some other changes and I dont believe it is easy to run an 03+ truck on a 99-02 PCM (fans IIRC).
