Problem with EFI v2 controler

When I bought me EFI live controller the screen had lines running vertically through it. I have tried getting the company I bought it from to fix it. They have told me several times they would send a return label and RMA. I am giving up on them after much to long of a wait.

Where do I send it to be repaired and what is the process. I am in the US.

Vertical lines can be adjusted using the screen contrast option.
