A guy I know had this issue and replace sensor but the new sensor was bad out of the box so he had to order another and it fixed it. If your sure your sensor is good, injectors are good and relief is good then check your wiring. Does it set code after sitting a while? Only when cold? Can you clear code after engine is running and keep code from coming back? I have read about a few trucks in the 11-12 model year with this issue and it ended up being an injector issue.

The architecture for setting this code is that it will only run the diagnostics after an 8 hour cold soak, must be at idle, and at operating temp. Takes 2 consecutive failures to set off CEL. So after clearing the code, it takes two days to reset. Diagnostics compare the rate of change in fueling with respect to the rate of change of pressure to a calibrated fail high threshold or above the calibrated fail low threshold while the monitor was running. Would really like to know what the actual calibrated thresholds are.

Not sure on that. Have the injectors ever been removed?
