When I went from stock os to the Cos3 2bar tune.
When I reflash my truck it will run for sec than die. I had to do this two or three times before it run on it's own. Do need to do a crank re-lean after you do this or this common thing.

YOU ONLY NEED TWO TOOLS IN LIFE - WD-40 AND DUCT TAPE. IF IT DOESN'T
MOVE AND SHOULD, USE THE WD-40. IF IT SHOULDN'T MOVE AND DOES, USE THE
DUCT TAPE.

Might just be your idle learning. Which might suggest that your base map might need some work
If its ok now Id say thats what the problem was. Look into the RAF and Idle transition tuning notes.

Every time it should stumble (MAF fails goes into SD) a little but should not fail to start and stay running .

Did you follow the COS tutorial...?
Did you do full flash of COS followed by cal flash of your old tune...?
Is your COS file compatible with your old tune as listed on the COS tutorial page 4...?
