From a quick search I can tell you that the 12615136 is the superceded number for the 12580698 2 bar sensor. Cant help you with what it is from though.

Number sounds familiar. Is it for a truck?
I have a feeling that's the same ne I'm using in my Burb.

YOU ONLY NEED TWO TOOLS IN LIFE - WD-40 AND DUCT TAPE. IF IT DOESN'T
MOVE AND SHOULD, USE THE WD-40. IF IT SHOULDN'T MOVE AND DOES, USE THE
    DUCT TAPE.
IF YOU CAN'T FIX IT WITH A HAMMER, YOU'VE GOT AN ELECTRICAL PROBLEM.
