This is a weird OS that had limited use, Im not aware of any manual trans that used this OS. I know the Tahoe/Yukon/Avalanche used it with both 60E and 80E, but none of those vehicles were made with a manual.
Is this in a Tahoe or a swap? Do you need FlexFuel support on a 512kb PCM or could you switch to an 03-05 PCM? Downside is you'll need to match the pedal/TAC/OS year so you might have to buy some new parts. If in a stock vehicle, you might have to switch to whatever OS that works with a compatible manual trans (I know 2000-2002 4.8 Silverados had a Manual option).

2008 TrailBlazer SS 3SS AWD Summit White - LS2 E67/T42, bolt ons, suspension, etc.
2002 Chevy TrailBlazer LT 4X4 Summit White - 4.2L I6 P10, lifted, wheels, etc.
Using 12216125 for the flex fuel on a 411 conversion.
As have all the bits really didn't want to have to buy another ECU and license.
May give it a go with all the auto setting changed to Manual and DTC turned off and see what happens.
A segment swap would make it easy-er.
Depending on what you need to work, you might have to change the trans type to manual. I have found this in other 411 OS's and could probably help you find it for the 12216125 OS. This would be a CAX file type change.
If turbo_bu can help you out, it'd be worth it. Basically know it will run in P/N the whole time so you'll have to be mindful of that as well.

Hey Apples, did you ever get this to work? I was trying to do the same on my car. Eventually just abandoned it and switched to straight E85 tune, but I would really like to get the flex fuel features working.
