I was bench flashing it and this was what I got on every time trying to flash. Is there any chance of recovering it? It was rebuilt about a year and a half maybe two years ago.

Was it recovered then flashed with TIS?
Was it like this before attempting to flash it? In otherwords was this the first time you tried to program it on the bench?

I've bench flashed this one a few times now over the year.
I'm not sure how it was recovered I sent it into SoCal to be fixed and has been great until last night.

This is starting to bother me. I have used HP Tuners 50-60 times on my truck and many others with no issues or having to pull fuses. I upgraded my EFI Live cable so I could try the DSP5 on my truck but it looks like EFI Live is very unstable on this LB7 platform. I have been using HP Tuners for over ten years on my truck :(
LB7's in general can be hit or miss, it's just how it is. I've flashed mine MANY times with NO issues over the last 7 years, as well as THOUSANDS of others, but LB7's can be a PITA if you hold your tongue wrong.
